Health Connect Initiative

  • Health Connect Initiative: An enhanced scattered-site housing model that provides mobile, interdisciplinary care coordination for individuals with complex behavioral health, medical, and social needs to improve housing stability, reduce crisis utilization, and strengthen coordination between housing and healthcare systems.

Transitional Housing Initiative for Justice-Involved Individuals

  • A structured reentry housing model that supports individuals returning from incarceration through transitionalhousing, case management, behavioral health coordination, workforce readiness, and pathways to permanent housing, promoting stability and reducing recidivism.

Housing Initiative-Social Practice Model

  • Developed a housing initiative integrating social practice, a non-clinical approachleveraging an intentional community.

  • Involved community members as active decision-makers in the social environment.

Housing Resource Center (HRC)

  • Established the first Housing Resource Center within the clubhouse model to provide comprehensive housing resources.

  • Facilitated collaborations with external housing partners, conducted staff training, and supported members facing housing instability.

Positive Housing for ALL

  • Managed a program providing social and medical services, including housing, to individuals with HIV/AIDS not eligible for HASA benefits.

  • The initiative concluded after the passing of the HASA for ALL Law by Former Governor Cuomo.

Housing Retention Program

  • Designed to assist individuals with HIV/AIDS in HASA SROs and hotels in acquiring permanent housing.

  • Continued providing support for one-year post-housing to ensure community integration and prevent recidivism.
